Assessment

An individualized, comprehensive biopsychosocial assessment of the patient’s substance use disorder, shall be conducted or updated by credentialed staff who are knowledgeable about addiction treatment.

This assessment is used to confirm the appropriateness of placement.

Individual Service Plan

We provide an individualized treatment plan, which involves problems, needs, strengths, skills, priority formulation, and articulation of short term, measurable treatment goals, preferences, and activities designed to achieve those goals. The plan is developed in collaboration with the patient and reflects the patient’s personal goals.

Physical Assessment

Physical Exam

A physical examination by a qualified practitioner at the initiation of treatment ensures that the patient is medically stable and appropriate for the low intensity residential treatment. This examination shall be conducted within a reasonable time upon admission into the program.

Medical Admission

Eligibility Criteria

The key to successful treatment is an initial evaluation and assessment of the patient and his or her family by our treatment professionals. Admission to the low intensive residential treatment is based on this assessment and meeting the following criteria:

  • Be at least 18 years old
  • Have a diagnosis of primary chemical dependency
  • Be sufficiently medically stable for this level of care and not manifest symptoms of withdrawal
  • Exhibit symptoms of an addictive disorder and symptoms of a psychiatric illness of such severity that there is significant interference with social, vocational, or educational functioning
  • Have the cognitive ability to benefit from insight-oriented therapy
  • Agree to periodic and random urine drug screens and breathalyzer or blood alcohol testing while participating in the program
  • Not be as distressed or disordered as to present an imminent danger to self or others
